Become a CHG Patient Evaluator
CHG needs skilled professionals to assess patients and determine demographic and placement information to better help the program serve the patient and the community / hospital.
Potential Patient Evaluators (PE) will possess:
- 10-years + guardianship experience, or equivalent
- No active guardianship cases in the geographic area where they will be a Patient Evaluator
- Comfortable with technology such as computers, email, Word, Zoom, etc.
- No OPPG registration issues
- Willing to sign a contract to serve as PE
- Review and agree to the PE duties prior to signing contract
- Willing to provide W-9
